How much does motorcycle shipping cost?

Usually 40-60% of a sedan on the same lane for open transport. Enclosed is a premium on top of that.

Should the bike be crated?

Not usually for domestic road transport. A proper chock-and-soft-tie setup in an enclosed trailer is the standard, and crating adds cost without much added protection.

Can I ship gear with the bike?

Small items in locked hard bags are generally fine. Loose gear is not covered and adds weight the carrier prices for.
